A State of Connecticut health form, which includes immunization information, will be completed with each routine annual physical exam and given to you for your records. Please keep this in a safe place and make copies to provide to schools, camps, and other institutions as needed. Additionally this form will be scanned in to your child’s patient portal for you to access at your convenience.
If you need additional forms completed by your physician, please submit them with a self-addressed stamped envelope, and allow 5-7 business days for completion. There may be an additional charge for this service.
